How to Inject
Epinephrine is the treatment of choice for all severe reactions following an immunotherapy injection.
🫀 Heart
- Weak pulse
- Dizziness
- Passing out
🫁 Lungs
- Shortness of breath
- Significant chest tightness
- Wheezing
👄 Mouth & Throat
- Rapidly advancing face/lip/tongue swelling
- Throat tightness or closure
- Difficulty swallowing or talking
🤢 Stomach
- Vomiting / Nausea
- Diarrhea
- Cramps
⚠️ Skin (Mild)
- Itching, redness, swelling at injection site
- Diffuse hives or flushing
- Welts beyond injection area
⚠️ Mild Reactions
- Treat with oral Benadryl® + ice pack
- Report to provider if >12 hours
- Do NOT place ice directly on skin
1. Assess — Anaphylaxis vs. Vasovagal?
Anaphylaxis: tachycardia, hives, breathing difficulty. Vasovagal: bradycardia, cool pale skin, no hives — lay flat, legs elevated, usually self-resolves.
2. Epinephrine — administer immediately
Use epinephrine auto-injector as directed. May repeat every 5–10 minutes. Keep patient supine with legs elevated. Do not allow patient to sit or stand.
3. Call 911
Inform dispatcher epinephrine was used. Do not discard auto-injector. Patient needs 4–6 hour observation at ER.
4. Oral Benadryl® (if able to swallow)
Take oral liquid Benadryl® 12.5 mg/5 mL per label instructions. ONLY after 911 has been called.
5. Tourniquet (IT-induced only)
Apply proximal to injection site. Release every 5 min for 3 min. Do not exceed 30 min total. Occlude venous — not arterial — flow.
6. Follow up
Continue oral Benadryl® q6h for 24–36 hrs and oral prednisone daily for 2–3 days as directed by provider. Follow-up appointment in 24–48 hours.
Stop immunotherapy if ANY of these are present: Fever ≥101°F · Sinus infection · Difficulty breathing · Asthma symptoms · Pregnancy · Recently prescribed antibiotics or beta-blockers
Never administer if: Vial labels don't match patient name · Vials have expired · Vials not stored appropriately
Storage: Vials → refrigerator 35.6–46.4°F (2–8°C), never freeze. Epinephrine → room temp, protect from light, NOT in a car glove box.
Timing rules: No injection within 2 hrs before OR after strenuous exercise. No injection within 3 hrs of bedtime. Keep epinephrine on hand for 3 hrs after each dose.
- Use two separate syringes — one for A vial, one for M vial. Never mix.
- Gather supplies: epinephrine auto-injectors, oral Benadryl®, dosage log, A & M vials, 2 new syringes, alcohol pads, sharps container.
- Wash hands thoroughly with soap and warm water.
- Wipe tops of both vials with alcohol; let air dry.
- Remove needle cover; insert one new needle into rubber stopper of each vial.
- Wipe injection site with alcohol in a circular motion outward from site; let air dry.
- Turn A vial upside down and withdraw the prescribed dose.
- Insert needle at a 45-degree angle with bevel facing away from skin.
- Aspirate (pull plunger back) before injecting. If blood appears — DO NOT inject. Remove at same angle; discard syringe; use new syringe; new site ≥1 inch away.
- If no blood — inject; remove at same angle. Discard needle immediately into sharps container.
- Repeat steps 6–10 for M vial injection.
- Date and initial the dosage log.
- Keep epinephrine on hand for at least 3 hours after injection.
